Trinity Health PACE of Alabama is Hiring a Registered Nurse Near Mobile, AL
Schedule: Monday-Friday, 8am – 5:00pm, with On-Call Weekend Rotation every 6-8 weeks.
Position Purpose:
- Provide skilled nursing care, treatments, and care plans to participants, ensuring adherence to professional standards and primary care orders.
Responsibilities:
- Deliver skilled nursing care and treatments to participants, emphasizing self-care education.
- Collaborate within an interdisciplinary team to assess participant health and develop care plans.
- Actively participate in daily program activities, promoting health and independence.
- Offer guidance and referrals to participants and families regarding service needs.
- Maintain accurate records and contribute to Quality Assessment and Process Improvement (QAPI) initiatives.
Minimum Qualifications:
- Graduate of accredited Nursing Degree program with active RN license.
- Two years of successful experience in related services with geriatric population.
- Valid driver's license and CPR Certification required.
- Ability to perform clinical procedures accurately and with empathy.
