GENERAL SUMMARY:

Under thedirection of a Program Clinical Supervisor, the Clinician is responsible forproviding professional therapeutic mental health services and consultation tolocal agencies and other professionals in the community.


ESSENTIAL JOBFUNCTIONS:

  • Provides counseling and/or therapy according to a plan of care developed from assessment and diagnostic evaluation.
  • Evaluates and assesses the holistic needs of the individual client.
  • Develops a written plan of care in conjunction with the client, the client's family where appropriate, and significant others in accordance with client/family needs and in compliance with agency procedures.
  • Utilizes the consumer's electronic health record (EHR) to ensure that documentation is complete, accurate, timely and meets regulatory and agency standards.
  • Carries out therapeutic treatments and procedures according to established care plans, agency and program policies and procedures, professional standards, and within the scope of practice of the employee's professional license.
  • Refers clients to other professionals or services as appropriate to ensure optimal service.
  • Arranges medication call-ins with various prescribing psychiatrists or nurse practitioners.
  • Discontinues services appropriately, referring for additional follow-up services from the health care community, when warranted.
  • Interacts with other agency programs for referrals, as necessary.
  • Develops, maintains and appropriately utilizes knowledge of community resources.
  • Provides education, professional consultation services as necessary.
  • Educates and instructs clients and significant others in prevention and rehabilitative measure appropriate for the client's care and safety.
  • May provide case management services including: street and community outreach; housing location/stabilization; and insuring the provision and maintenance of basic needs (food, clothing, etc.).
  • Assists clients in developing the knowledge and skills and acquiring the necessary supports to live as independently as possible in the least restrictive setting in the community. This includes: budgeting/money management, hygiene, housekeeping, nutrition, shopping, representative payeeships as needed and community integration activities.
  • Links to and provides support with other services (vocational, educational, social/recreational, transportation, etc.) and advocates for entitlements such as Mainecare.
  • Provides interface between the ACT Team and other service providers throughout the state to specific clients.
  • Assists clients in obtaining non-psychiatric medical care.
  • Provides coverage and works shift assignments which will include some after hours, weekends, and holidays.
  • Provides education with consumers around symptom management, substance
    abuse, etc.
  • Uses personal vehicle as may be required for consumer transportation, outreach in the community, home visits, and/or agency related business.

EDUCATION ANDEXPERIENCE REQUIRED:

  • Duties require aMaster's Degree in Social Work, Psychology/Counseling or PsychiatricNursing. Three to five years of directly related experience preferred. 
  • Must possessLMSW/LCPC/LMFT conditional clinical license or APRN-CNS clinical licensure withPMHCNS-BC certification. Clinicians that are dually licensed as a CADC orLADC are preferred.
  • Experience working withand knowledge of co-occurring disorders preferred.
  • Must possess a validState of Maine driver's license if required to travel.
