Job Responsibilities:
1.Develop wound care program in partnership with the Director of Nursing and consultant Physician.
2.Monitor and assess any new skin observation alerts, recommend treatment.
3.Develop nursing care plans for resident at risk or currently with skin integrity issues, include treatment protocols.
4.Provide resident and staff education. Work collaboratively with Physical therapy and nursing staff to maintain skin care programs.
5.Supervise and direct the activities of nursing personnel to ensure the provision of wound care to the residents of facility.
6.Assess residents to determine their risk factors for contractures, skin breakdown and deterioration in ADL’s in order to develop a nursing rehab plan of care.
7.Monitor the effectiveness of the plan of care through direct observations of care rendered and through weekly wound rounds/ reassessment.
8.Develop plan of care for resident with a current pressure sore, or evaluated as high risk for the development of pressure sores.
9.Provides reports on the type and frequency of wound in the facility.
Required Qualifications, Skills and Abilities
1. Education: Completion of a formal education program in Nursing
2. License/Certification: Valid NYS RN License, CPR certified
3. Equipment/Computer Software: Basic computer/ EMR software
4. Physical/Mental Demands: Walk long distances, climb stairs, run in an emergency and remain on her / his feet for long durations of time. Bend, squat, push, pull, reach, stretch and lift up to 50lbs. Speak, read and understand English. Hear the spoken word. Demonstrate hand/eye dexterity for operating ADL and medically complex equipment, e.g. oxygen concentrator, I.V. equipment, parenteral feeding equipment, peritoneal dialysis equipment, ventilator equipment, etc.
