The Case Manager assesses treatment needs; develops, monitors, and evaluates progress; and facilitates interdisciplinary approaches. The Case Manager will complete an assessment of the individual’s strengths and needs to work with the individual to complete an individualized strength- based service plan to guide them in overcoming barriers to recovery and towards independence and stability.

A minimum Bachelor’s degree in related field is required; a Master’s Degree in related field is preferred. It is strongly desired the individual have 4 or more years' experience and/or education in the behavioral health field. Experience in providing case management is strongly desired. Licensed Social Workers and Certified Addiction Specialists are encouraged to apply. Bilingual/Multilingual applicants are strongly encouraged to apply.
