Supports Coordinator (Case Manager)

Supports Coordination is the service of developing, identifying, coordinating, monitoring and maintaining supports and services for individuals with intellectual disabilities and/or autism and their families. The primary focus is to assist individuals in achieving outcomes and to advocate for an Everyday life. To do this, you will serve as a liaison between the individual, family, community agencies and services, the county, and the state.

Duties:

  • Engage individuals and their families in the development of plans that assure the needs of the individual are addressed, and life opportunities expanded.
  • Assist, coordinate, and monitor the delivery of chose supports and services.
  • Promote individuality, choice and control.

Requirements:

  • You must have a strong belief that people with disabilities can achieve outcomes, have value, and contribute to their community
  • You must be proficient in the use of computers
  • Must have a car
  • Bilingual (Spanish/English) proficiency is a plus!

Job Type: Full-time

Pay: $35,600.00 - $41,600.00 per year
