Position Title: Intake Technician

• Completes orientation and intake for all new clients which includes, but not limited to, property inventory, EHR intake and documentation, food stamp application and orientation to the unit.
• Maintains the Triage Board.
• Assists with discharge coordination when necessary.
• Collaborates with internal programs and staff to ensure appropriate handoff of care.
• Documents client data according to policy under direct supervision of RN/NP or designated clinical professional.
• Provides clerical support and coordinates communication to support the function of the Triage unit.
• Demonstrates exceptional interpersonal skills, including telephone, electronic, and in person. Relays clear, concise information, uses courtesy and tact in relating to clients/visitors, employees and providers and escalates issues when appropriate using established protocols and the chain of command.
• Attends and participates in unit/department meetings per unit policy and participates in required programs.
• Participates in quality improvement studies and projects as directed.
• Assists in other Transitions’ programs/tasks as needed or assigned.
