Transitions LifeCare is looking for an RN Case Manager to join our team!

We are growing and looking for competent, caring Nurses to join our professional and compassionate team. This is a full time position Monday - Friday, 8am/5pm. Will also rotate with the other CM's through a Back Up On Call schedule (paid at an additional rate).
Must be able to work independently and also as a vital part of our IDT Team as we provide care for our patients. Team consists of an RN, LPN, Chaplain, Social Worker and Aides.
