Transitions LifeCare is Hiring a Hospice Home Evening Clinical Team Leader (RN), Full-time Near Raleigh, NC

Transitions LifeCare is hiring immediately. As one of the community’s industry leaders, we provide exceptional home health, hospice, and palliative care services throughout our 8-county territory. Transitions LifeCare’s goal is to care for individuals and families with compassion, support, and expertise as they navigate the end of life or life changing illness.

The schedule for this position is Monday-Friday 3:30pm-12:30am; weekends and holidays as needed

A Successful Hospice Home Clinical Team Leader has the following:

Responsibilities:

Coordinate interviewing/hiring and orientation of new clinical staff.
Direct, prioritize and coordinate delivery of patient care, on the assigned shift, for Hospice Home patients.
Coordinate and problem-solve patient care issues with patients, families, physicians, agency staff and other health care providers.
Ensure staff documentation of patient care is complete, timely, and accurate in accordance with TL processes, policies, procedures and regulatory requirements.
Manage various personnel actions including, but not limited to hiring, orientation, performance appraisals, promotions and scheduling; and counsel and conduct formal discipline of clinical staff.
Develop and implement processes to enhance clinical operations and practices for ongoing quality improvement.

Requirements:

Registered Nurse with current NC license; BSN preferred.
Two years of previous supervisory experience preferred.
5 years recent clinical experience with hospice, home health, or equivalent.
Must have reliable transportation
Must submit and pass all applicable pre-employment and annual tests and meet all training and other applicable state and federal guidelines
