As a Manager of Bereavement Services at Transitions Care you will be responsible for determining the level of care and support of bereaving families and significant others following the death of a hospice patient. The goal of this role is to help the family/significant other cope with death related to grief and loss issues utilizing the necessary resources available, such as volunteers, social workers, spiritual care coordinators and more. Travel as necessary.

Major Responsibilities

  • Provide pre-bereavement counseling services per the Hospice Plan of Care.
  • Assess on an ongoing basis the bereavement needs and coping abilities of survivors.
  • Coordinate the delivery of grief counseling needs and services (one-to-one, groups, and ongoing follow-up) to ensure timely and appropriate services are provided to family/caregivers.
  • Utilize a bereavement tracking system in order to closely follow all families in a timely fashion. Submit documentation promptly as described in company policy.
  • Develop, conduct, and document bereavement volunteers training programs and continuing education for volunteers. Work with Volunteer Coordinator to assign volunteers to bereaving families. Evaluate the effectiveness of the Volunteer assigned and keep Coord Volunteer Services informed of performance.
  • Plan, develop, and lead bereavement support groups for survivors and staff members.
  • Able to locate and utilize resources for helping the families deal with their loss (books, brochures, community groups, etc.)
  • Organize or conduct a memorial service on an annual basis for all bereaved family members.
  • Identify the special needs of discrete populations of bereavement clients (e.g. children, persons with AIDS, young widows) and tailor these programs to meet these needs.
  • Recognize when complicated grief and/or coexisting psychiatric conditions pose a threat to client health or safety and initiates referrals for more intense psychotherapy and/or chemical dependence treatment as applicable.
  • Participate in staff meetings, department meetings, team meetings, briefings, in-services, committees, and other related activities as needed.
  • Participate in evening/weekend on-call as required, conducting on-call services in a clinically competent and responsive manner.

Requirements

Education and Experience

  • Bachelor’s degree in social work, counseling or equivalent theological degree from college or university, and certification of special training in bereavement.
  • Master's Degree preferred.
  • Three (3) years’ experience with families/caregivers and patients dealing with grief or in dealing with loss.
  • Identifies clients at risk for complicated grief. Recognizes signs and symptoms of chemical dependence and other psychiatric illnesses.
  • Ability to establish therapeutic relationships with clients from a variety of cultural, religious/spiritual, and socio-economic backgrounds.
  • Good working knowledge of community resources that can be used for appropriate referrals of bereaved family members.
  • Knowledge and commitment to hospice philosophy.
