As a Care Coordinator at Transitions Care you will develop new customer accounts and gain referrals / admissions within your territory.
You will establish and maintain professional relationships with all referral sources, including physicians, nursing homes, assisted living facilities and hospital personnel.
You will also interact with Transitions patient care management and staff while representing Transitions Care and its services in a competent professional and responsive manner.
