TRAC Transition Resource Action Center is Hiring a Transitional Housing Case Manager Near Fort Worth, TX

POSITION SUMMARY

The TRAC Residential Case Manager will provide case management services, support services, and independent living skills training to current and/or former youth ages 18 and older transitioning from substitute care to independent living. He/she will provide supportive counseling; linkage to resources; housing, employment and independent living training; crisis intervention and management, among other services, to participants in this program.

QUALIFICATIONS

Education: Bachelor’s degree required; degree in social work or a related field preferred; Master’s in Social Work (MSW) highly desirable.

Experience: Two (2) years of experience working with children or youth who are at-risk, in foster care or who have been neglected or abused.

Additional Requirements: Must be proficient in MS Word, Excel and Outlook. Must be proficient in managing a case tracking system. Must be available to work evenings, weekends, and on-call (24 hrs). Must have driver’s license and reliable transportation. Must be able to travel within assigned region on a daily basis.

PRIMARY RESPONSIBILITIES

This section describes the primary responsibilities of an employee in this position. Please note: The primary responsibilities of this position are not limited to this list.

1. Utilize the TRAC case management process to complete client intake, assessment, and individual transition planning based on crisis or planned intervention referral.

2. Provide direct services of supportive counseling; linkage with resources; assistance with housing, employment and independent living training; and crisis intervention services in alignment with transition plan goals.

3. Review and update the transition plan progress with assigned youth at least quarterly and more frequently as needed.

4. Assess youth’s progress toward self-sufficiency and complete the formal assessment as assigned.

5. Assist youth with planning for and/or locating safe, appropriate and affordable housing.

6. Assess individual financial needs of youth and document monthly. Complete documentation and requests for financial assistance for youth who require it.

7. With the goal of a livable wage career, assist youth with connecting to employment by helping them get on track with education, vocational training, employment training, and work or volunteer experience. Refer to local workforce programs, training opportunities or job coach for additional services and support.

8. Assist youth with improving their money management skills in budgeting, banking, saving, and planning for future needs. Refer to training opportunities or budget coach for additional services and support.

9. Assist youth in utilizing a wide range of community resources aimed at supporting their transition to independence, including cultivating caring adult relationships.

10. Provide practical application of the six elements of the training curriculum taught during the Preparation for Adult Living life skills classes, with primary emphasis on: Housing/Transportation, Job Skills, Financial Management, and Life Decisions/Responsibilities.

11. Assist youth with identifying affordable, appropriate, and accessible health care providers in their community.

12. Determine if service and skill building needs are best addressed with individual coaching, in small group settings or in class settings. Provide individual coaching, make referrals or plan group training as needed. Provide hands-on training to youth; this includes but is not limited to:

a. Take youth to a bank: help them set up an account, set up a budget and learn how to save money.

b. Take youth to prospective employers: help them fill out an application, prepare for an interview and tour the facilities.

c. Teach youth how to access mass transit systems and help them practice that mode of travel.

d. Take youth to apartment complexes: teach them how to apply for housing and maintain housing.

13. Contact at least twice per month each assigned youth. At least monthly, make face to face meetings with each assigned youth. Youth identified as “in crisis” must receive monthly face to face contact.

14. Document detailed information in client database system daily, including monthly summaries for each assigned youth.

15. Conduct client satisfaction surveys as requested for program reporting needs.

16. Verify youth eligibility for various benefits and services, as necessary for program compliance.

17. Conduct and facilitate training as assigned.

18. Participate in various TRAC events as requested by supervisor and/or to support clients.

PHYSICAL, MENTAL, AND SPECIAL DEMANDS

1. Ability to work a 40 hour week.

2. Ability to sit for long periods, up to 8 hours.

3. Ability to use both hands in fine and gross manipulation of small tools (copier, computer, telephone).

4. Ability to push and pull up to 10 lbs. (File cabinet drawers, computer paper boxes).

5. Ability to see well enough to read handwritten and typewritten material.

6. Ability to hear and speak well enough to communicate via phone.

7. Ability to lift and carry up to 25 pounds dead weight to move or transfer boxes or documents.

8. Ability to retrieve files from bottom shelves/drawers and reach up for items located on top shelves.

9. Ability to reach, turn and twist above and below the waist daily to retrieve files or supplies.

10. Ability to stand and walk daily to retrieve forms/supplies for other departments.

11. Ability to spend 10% of working time outside in temperature varying from -0oF to 110oF.

12. Ability to spend 90% working time in environment of continuous moderate voices and office machine noise typical for business office atmosphere.

13. Ability to handle a variety of repetitive tasks.

Job Type: Full-time

Pay: $38,475.15 - $45,000.00 per year
