Touchstone Mental Health is Hiring a PATH Grant - Program Coordinator Near Minneapolis, MN

Program Coordinator – PATH Grant

Starting Pay: $25.00 an hour

Location: Minneapolis (Seward)

Schedule: Weekdays (Five 8-hour days or Four 10-hour days)

The Program Coordinator oversees the data collection and data submission of information required for the Projects for Assistance in Transition for Homelessness (PATH) Grant.

Program Coordinator Responsibilities:

  • Uses reports generated in the electronic health record (Credible) to compile data.
  • Enters data into the HMIS system following established timelines.
  • Follows grant requirements regarding formats and deadlines.
  • Coordinates with the finance team regarding grant expenses.
  • Coordinates with the Electronic Health Records Specialist in creating reports in the electronic health record (Credible).
  • Supports the staff members providing services as part of the PATH grant.
  • Participates in staff meetings for community mental housing services.
  • Provides front desk coverage for the Administrative Assistant on a weekly basis.

Program Supervisor Requirements:

  • Experience providing administrative support required.
  • A strong attention to detail required. 
  • Experience working with grant requirements and deadlines strongly preferred.
  • Experience working with reporting functions in an electronic health record preferred.
