Position: Housing Stability Client Services Specialist
Reports to: Access and Housing Engagement Manager
Compensation Status: Full-time, Salary
POSITION PURPOSE: The Housing Stability Clinic Client Services Specialist primary duty is to assist qualifying participants applying for identification, transportation, and SNAP benefits at the Housing Stability
Clinic.
ESSENTIAL JOB FUNCTIONS:
· Meet with participants during set days of walk-in hours for ID appointments.
· Assist participants with procuring necessary
documents and services such as identification cards, birth certificates and
SNAP benefits.
· Assist participants with applications for birth certificates, if needed for housing or to obtain state ID.
· Submit applications and payment for services.
· Meet with participants during set days during walk in hours for bus pass screening.
· Purchase metro bus tickets.
· Track participants that receive metro bus tickets along with the amounts of metro bus tickets they receive in the program in HMIS and the Service tracker excel sheet.
· Meet with participants during set days during walk in hours for SNAP applications.
· Track SNAP applications and participants in the program in HMIS and the Service tracker excel sheet.
· Verify participants accessing ID and transportation services are at ZERO income.
· Verify integrity of data by comparing it to source documents to ensure participants qualify for services.
· Maintain participant-related data tracking systems, including case notes and complete HMIS entries in a timely manner.
· Responsible for accurately entering important organizational and participant data and keeping records up to date in Clarity and program excel sheets.
· Manage participants' documents by performing proper documentation, record keeping and informing participants when documents arrive, in a timely manner.
· Review data for errors, missing pages, or missing information and resolve any discrepancies.
· Exit participants from programs in Clarity in a timely manner.
· Maintain a satisfactory level of quality and productivity per HMIS and organization standards.
· Planning regular follow-ups to assess progress and additional needs for obtaining identification.
· Organization to manage several different cases at once.
· Continuously develop and utilize knowledge, skills, and competencies necessary to perform position responsibilities in a high-quality fashion and consistent with organizational, accreditation, and professional standards.
· Provide individualized, strength-based support, education, coaching, information and advocacy services.
· Provide on-going case management when applicable.
· Provide referrals to onsite and off-site partner agencies when appropriate.
· Complete internal and external referrals, as needed, for supportive services and work collaboratively as a team to advance participants' goals.
· Maintain a filing system and protect confidential participant information.
· Ability to maintain effective working relationships with individuals of diverse backgrounds including persons with mental illness, behavioral health and physical healthcare professionals, family members, social service providers, and public at large.
· Attend collaborative meetings, as requested.
KNOWLEDGE AND SKILLS:

· Knowledge of the principles of file systems, case files and records management.

· Skilled in working within deadlines to complete projects and assignments.
· Skilled in establishing and maintaining effective working relations with co-workers and other agencies employees.
· Skilled in operating a personal computer utilizing a variety of standard software.
EDUCATION AND EXPERIENCE:
· Minimum of A.S. degree in Human Services or related field and/or one to three years/experience working in human services; prefer experience working directly with people experiencing or at risk of homelessness.
