Responsibilities

• Providing services in accordance with policies and procedures and regulations that define scope of practice
• Prepares clinical and progress notes on the relevant and specialized nursing services he/she provides
• Assisting the RN and/or physician in performing specialized duties related to end-of-life care
• Assisting the RN in carrying out the plan of care
• Preparing equipment and materials for treatment, adhering to aseptic techniques as required
• Assisting the patient in learning appropriate self-care techniques as required
• Assessing patient/family response to care
• Insuring communication of information to appropriate team members

Qualifications

• New York Licensed Practical Nurse (LPN) license, currently active and in good standing
• Reliability, professionalism, and punctuality are strict requirements
• Must have reliable means of transportation to and from work
