Job Description

Position Title: Case Manager, In-Home Supports Program

Department: In-Home Supports

FLSA Status: Exempt

Reports to: Director of Programs

Position Summary:

Responsible for helping the individual and his or her family members understand the services and supports that he or she may be eligible for from the Agency. Works with Support Coordination agencies to provide information about available services and supports so that the individual and the family can make informed choices regarding services. Explains services and assists in the process of determining what services will best meet the needs of the consumer.

Essential Job Functions:

  • Oversees assigned cases within the In-Home Supports Program, including supervising the Direct Support Professionals working one-on-one with Disability Allies clients.
  • Participate as a member of an inter-disciplinary team in the development and implementation of service plans for the individuals served.
  • Implement Individual Service Plans, including updating and maintaining Electronic Health Records.
  • Monitors cases by verifying clients’ attendance; observing and evaluating treatment plan and responses; advocating for needed services and entitlements; obtaining additional resources; intervening in crises; and providing personal support.
  • Maintains clients’ records by reviewing case notes, logging events and progress; provide oversight and responsibility for client I-Records.
  • Communicates clients’ progress by conducting weekly interdisciplinary meetings and evaluations; disseminating results to therapeutic team and family; removing obstacles and identifying treatment influences.
  • Responsible for orienting family members to services and providing resources as needed.
  • Acts as liaison between the family members and the Direct Support Professional.
  • Provide on-going support to families and the assigned Direct support professional.
  • Makes sure the individual is safe and healthy.
  • Performs outreach to determine if the person and his/her guardian are satisfied with services.
  • Reviews the ISP, ongoing services and supports regularly.

Minimum Qualifications:

  • Bachelor’s degree in social services, public health, or related field preferred.
  • Minimum 3-5 years’ experience in case management.
  • Must possess strong written and verbal communication skills.
  • Must have the ability to work effectively across all agency lines.
  • Must possess excellent interpersonal and customer service skills.
  • Must demonstrate the capacity to work effectively with electronic health records.
  • Experience with Therap Services a plus.

Travel - 50%
