The Whole Person is Hiring a LPN In Home Near Kansas, MO

GENERAL DESCRIPTION

This position will be responsible for performing nursing assessments of clients to determine in-home care needs.

ESSENTIAL JOB FUNCTIONS

  • Performs Completes authorized nurse visits for clients per DHSS care plan
  • Comprehensive assessment collecting health information, vitals, meds, and allergy information.
  • Performs General Health Evaluation every 4-6 months per DHSS Plan of Care clients.
  • Assists in signing off on point-of-service nurse visits.
  • Provides fill-in nurse visits when point-of-service is unavailable.
  • Assist in providing oversight/evaluation of Patient Care Assistants.
  • Assists in case management duties per DHSS regulations
  • Complete Nurse visit notes during visit time
  • Coordination of care with primary care physicians with any change requests
  • Coordination of care with Nurse Manager with any missed visits or client changes
  • Regular and predictable attendance

MINIMUM REQUIREMENTS:

  • Graduated from an approved school of training for the LPN program
  • LPN Certification Active Missouri LPN License
  • Two or more years as an LPN or RN in a health related field
  • Good Driving Record
  • Maintain active car insurance
  • Maintain active Missouri LPN license

  • Ability to maintain confidentiality
  • Good computer skills (Microsoft Office)
  • Good customer service skills
  • Able to follow through with a task and meet deadlines
  • Good written and verbal skills
  • Positive Attitude

PHYSICAL DEMANDS

To perform this job successfully, an individual must be able to stand and/or sit for long periods of time. The individual must be able to use hands frequently. The individual must be able to talk, hear, climb, reach, bend, twist and/or turn, stop, kneel, crouch, crawl, see, taste and smell. The individual must be able to push, pull, lift, and carry at least 50 pounds. Specific vision abilities required by this job include close vision, distance vision and color vision. May be required to assist a client with mobility and transfers, with or without assistive devices.
