Intensive Case Management Supervisor

DUTIES AND RESPONSIBILITIES:

  • Maintains the responsibility along with the Manager of Support Services for ensuring successful implementation/completion of ICMS contract and services within required scope of services
  • Responsible for maintaining successful data integration of all clients within both CHAMP & HMIS Systems
  • Will provide direct supervision to all ICMS staff, providing bi-weekly individual supervision, supporting in clinical supervision, and case conferencing.
  • Will oversee a caseload of ICMS participants.
  • Will ensure clients remain eligible and compliant with all required paperwork as required by the Housing Authorities as to not loose subsidized housing.
  • Responsible for leading any efforts that require changes or improvement to our current program policies and procedures.
  • Ensures all clients are receiving high quality service delivery and program is offering most relevant evidence based approached to homeless and health services
  • Will maintain appropriate reports on clients enrolled through our DHS and LACDA contracts; will provide most relevant information to Manager of Support Services on an ongoing basis
  • Provide reports on a monthly basis regarding client engagement and housing status updates
  • Responsible for onboarding and training of all new staff
  • Develop new relevant training materials as needed in accordance with COA standards and contract requirements.
  • Promotes alignment between housing, mental health, and health services.
  • Provides direct support to staff, conflict resolution, problem solving, crisis support, and coaching on activities of daily living to clients and families; will serve as emergency crisis contact for ICMS clients
  • Assists in managing crisis situations and grievance processes that adhere to safety and housing practice protocols
  • Responsible for ensuring both client electronic and hard files are audit ready and compliant with required contracts
  • Attends meetings and participates in learning networks related to housing and health services
  • Supports all existing agency policies, principles, and mission
  • Will provide support on an as needed basis with special projects within the umbrella of Support Services
  • Other related duties as assigned and approved by the Manager, Housing Support Services

KNOWLEDGE, SKILLS AND ABILITIES:

  • Extensive knowledge of Intensive Case Management Services, and subsidized housing settings and social service systems
  • Knowledge of Public Health, Mental Health and Homeless Services Programs and Trends
  • Knowledge of the Los Angeles Continuum of Care, Department of Health Services, HMIS, & CHAMP Systems
  • Knowledge of methods and principles of medical care, disease and disability prevention, health promotion and medical rehabilitation
  • Ability to establish strength based and positive relationships to work cooperatively with individuals, groups and organizations that are diverse in composition
  • Ability to manage budgets
  • Demonstrated ability providing direct supervision
  • Strong written communication, verbal communication, and presentation skills
  • Demonstrated organizational skills and proven ability to work independently and manage multiple projects and priorities within a multidisciplinary team environment
  • Self-motivated to produce quality results, learn concepts and develop skills necessary to work towards a goal
  • Excellent people skills, specifically customer service skills and a capacity for collaboration and interpersonal relationships. Will have the ability to mobilize staff/groups towards common goals and attain deadlines
  • Flexible, acute problem-solving skills, and the ability to work under competing deadlines in a fast-paced environment and adapt to changing work demands.
  • Bi-lingual (English/Spanish).

QUALIFICATIONS:

  • Master’s degree in social work, psychology, or other social services related discipline.
  • Minimum 3 years providing direct supervision, and managing teams.
  • Minimum 5 years of direct service and experience in social services field, preferred Housing/Homeless & Health Services.
  • Requires California driver’s license, personal vehicle, and proof of personal vehicle insurance coverage and the ability to drive for work.
  • Provide proof of full COVID-19 vaccination.

WORKING CONDITIONS:

Work is performed in a normal office setting. The noise level is moderate with occasional loud outbursts. Occasional need to work out in the field contacting clients in homes and schools. Frequently uses motor vehicle to effectively execute job duties.

PHYSICAL DEMANDS:

  • Must be able to remain in a stationary position 50% of the time
  • Needs to occasionally move about inside the office to access file cabinets, office machinery, etc.
  • Constantly operates a computer and other office productivity machinery, (i.e., calculator, copy machine and computer printer).
  • Constantly converses with staff and clients
  • Frequently operates a motor vehicle to travel to school or home site
  • Frequently needs to position self in order to effectively interact with children of lesser stature
  • Occasionally needs to transport materials (food, clothing, books, etc.) weighing up to 50 pounds for therapeutic activities
  • Frequently needs to use motor vehicle for business purposes.
