Job Description:

The Vistas at Bettendorf is looking for a detail-oriented MDS Coordinator to join our healthcare team. The MDS Coordinator will be responsible for:

Responsibilities:

• Ensuring accurate and timely completion of the Minimum Data Set (MDS) assessments and care plans for our patients
• Conduct comprehensive assessments of patients' medical, functional, and psychosocial needs
• Coordinate with healthcare providers to gather necessary medical documentation
• Utilize clinical documentation improvement strategies to ensure accurate coding and billing
• Collaborate with the interdisciplinary team to develop individualized care plans
• Monitor and update patient records in compliance with regulatory requirements
• Provide education and training to staff on MDS completion and documentation best practices
• Prior experience in HEDIS reporting and medical documentation
• Knowledge of hospice care, case management, and utilization management practices
• Understanding of Diagnosis Related Groups (DRG) and reimbursement methodologies

Requirements/Qualifications:

• Must possess a current, unencumbered, active state license to practice as an RN
• Experience in Skilled Nursing/Rehabilitation facilities preferred
• Six (6) months of experience as a MDS Coordinator
• RAC-CT or RNAC required
