Job Description

Want to work with a team that not only cares about their residents but for their team as well?

We are looking for an LPN to care for our residents in a professional and compassionate manner. The ideal candidate will be patient and friendly with excellent communication skills.

Monitor residents for changes in condition, behaviors and signs/symptoms of illness and effectively communicate these changes to the supervisor.

  • Partners with the healthcare staff, Administrator and RN to maintain compliance with state regulations and The View Communities protocols.
  • Conducts retention activities to ensure adequate levels of engaged team members
  • Presents at team members meetings as requested by Administrator.
  • Responsible to conduct resident rounds daily to ensure overall resident care.
  • Provides team member training as requested by the Administrator.
  • Responsible for the development of individualized service/care plans to meet the needs of each resident.
  • Performs all necessary LPN duties as required by state regulation.
  • Conducts investigations of incidents and reports of caregiver misconduct to ensure accurate and timely completion. Reviews reports to identify trends for monthly QA meetings.
  • Oversees medication program to include maintaining safe storage of medications, observing medication passes, monitoring the medication error program and conducting periodic medication reviews.
  • Supports and assists marketing and move in process.
  • Communicates concerns and on-going health status to physicians, families and caseworkers in a timely manner.
  • Conducts initial assessment on prospective residents, including off-site visits as needed within 23 hours of referral.
  • Provides education on various health topics including disease prevention and self-care to residents on an as-needed basis.
  • Responsible to ensure that the written resident record that pertains to health assessment, health monitoring, services planning and personal care progress notes are current and accurate.
  • Manages and provides clinical measures to the home office for analysis and process improvements
  • Provides general and specific job orientation to all newly employed resident assistants.
  • Ensures the facility's infection control program is communicated and current.
  • Conducts assessments as follows to assess resident appropriateness for continued placement:
  • Semi-annually at change of condition or by family/resident request
  • Participates in the resident care conferences.
  • Assists in preparing, controlling and meeting or exceeding operating and capital budget.
  • Promotes and ensures compliance with the company's safety program.
  • Models the Mission, Values, and Principles of Service with our Residents and fellow Team Members
  • Ensures that department team members are compliant with training standards
  • Performs other duties as assigned

Requirements

  • Moderate level of computer knowledge: Data base management, e-mail, Word, Excel, Outlook/iCal, and web based programs
  • Ability to occasionally lift up to 50 lbs.
  • Ability to tolerate up and down movement including reaching above and below shoulder height and work to manage lift.
  • Ability to communicate orally and in writing with residents, families, and coworkers.
  • Ability to be on one's feet for extended periods of time.

Proven experience as an LPN
