Job Description---

Medical Assistant (MA) Population Health

As a member of our newly formed Population Health department the medical assistant role is to support the team in providing a proactive approach in the prevention and treatment of problems that could put a patients’ health at risk and/or problems that could be considered barriers to successful plan implementation. The medical assistant will provide administrative support for the care management activities.

Representative Duties:

• Facilitate hospital and other records into the EMR prior to visit.
• Initiate standing orders and medication refill requests.
• Call patients for ER follow-up and schedule follow-up visit within the designated timeframe.
• Assist RN Health coaches in coordinating care team services.
• Obtain records from external healthcare sources and/or EHR review to support coordinated patient care and to close documentation gaps.
• Complete follow up care management calls with patients.
• Participate in team huddles, care management planning and departmental meetings.
• Participate in continuous quality improvement activities and initiatives.
• Provide educational information to patients, family, etc.
• Collect information from external physicians and hospitals.
• Call patients for ER follow-up and schedule follow-up visit within the designated timeframe.
• General administrative responsibilities as directed supervisor.

Job Requirements---

Job requirements:

• Certified or Registered medical assistant
• Healthcare experience
• Use AIDET (acknowledge, introduce, duration, explain, and thank-you) and manage up (promote colleagues).
• A passion for helping others
• Strong work ethic
• Articulate and creative in verbal communication
• Proficient in MS Office and experience in medial office scheduling
• Maintains patient confidentiality
• Adhere to the OUHP/SCM Standards of Behavior

Education:

• High School diploma or GED or completion of an accredited Medical Assistant Program.

KNOWLEDGE/ SKILLS/ ABILITIES/ AND OTHER CHARACTERISTICS:

• Advanced knowledge of medical terminology.
• Ability to interact professionally with healthcare practitioners and patients.
• Ability to supervise.
• Ability to speak fluent English and Spanish.
• Ability to read and write English and Spanish.
• Ability to work independently and as a team player.
• Must be detail oriented and complete work tasks.
• Ability to maintain composure even in difficult situations.
• Ability to be understanding and sensitive to needs of others.
