Overview:
We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for providing support and guidance to individuals and families in need. Your role will involve assessing client needs, developing care plans, and connecting clients with appropriate resources and services. Responsibilities:
- Conduct assessments to determine client needs and develop individualized care plans
- Provide counseling, mentoring, and support to clients in various areas such as social work, childcare, crisis management, addiction counseling, and behavioral health
- Assist clients in accessing community resources and services, including Section 8 housing assistance
- Collaborate with other professionals, such as healthcare providers and social workers, to coordinate care for clients
- Monitor client progress and make necessary adjustments to care plans as needed
- Provide crisis intervention and support during emergency situations
- Supervise and guide clients in developing life skills and achieving personal goals
- Utilize motivational interviewing techniques to empower clients to make positive changes

Experience:
- Bachelor's degree in social work or related field ( degree preferred)
- Proven experience in case management or related field
- Knowledge of community resources and services
- Familiarity with crisis intervention techniques
- Strong communication and interpersonal skills
- Ability to work effectively with diverse populations
- Excellent organizational skills and attention to detail
