Responsibilities:
The Hospice Admissions Nurse (RN) will:
- Present hospice services to patients and families is individualized, sensitive to patient and families fears, needs and goals,
- Perform comprehensive assessments to identify all physical, psychosocial, environmental, safety, emotional and spiritual needs related to the terminal illness that must be addressed in order to promote the patient’s well-being, comfort, and dignity throughout the dying process and documents findings appropriately in the EMR
- Perform pain assessments using, adhering to pain protocols, knowledgeable in equi-analgesic dosing; optimal pain relief using an interdisciplinary focus and timely interventions
- Articulate clinical course/disease progression of cancer(s), lung disease, heart disease, liver disease, dementia, neurologic disease states
- Initiate the plan of care timely and appropriately with a demonstrated skill in anticipating problems that may cause or create a threat to patient safety or lack of symptom control
- Perform other duties, as needed
- Graduate of an accredited School of Nursing
- Current NYS Registered Nurse (RN) license
- Solid assessment, clinical, and documentation skills
- Working knowledge of Electronic Medical Records (EMRs)
- Microsoft Office Suite proficient
- Solid assessment, clinical, and documentation skills
- Great interpersonal skills
- Excellent communication skills (written and verbal)
- Strong attention to detail
- Highly organized
- 2 years of Nursing experience
- Bachelor's Degree in Nursing
- Certification in Hospice and Palliative Care Nursing
