Job Title: Social Work Case Manager Site Coordinator
Location: Baltimore, MD
Job Summary: 
The Social Work Case Manager (SWCM) Site Coordinator in Baltimore, MD, is a pivotal role responsible for providing direct support and communication to the site medical team. The primary focus is to deliver comprehensive social work and medical case management services to patients and families, addressing social determinants of health impacting access to and engagement in medical care. Additionally, the SWCM Coordinator manages specialty care internal referrals and oversees student interns at the site.
Major Duties and Responsibilities:
Planning and Organizing:
- Ensure effective communication with site medical staff, implementing care requests in line with organizational and regulatory standards.
- Contribute to the development and implementation of psychosocial interventions aligned with organizational strategies and medical/grant deliverables.
- Plan, coordinate, and participate in case conferences with multi-disciplinary teams.
- Organize and track all internal referrals, serving as the JHH TAP Program Navigator for the site.
Checking, Examining, and Recording:
- Maintain project reports, documents, and data, including Tableau reports on internal referrals.
- Conduct audits of follow-up on internal referrals, reporting findings to the Director of SWOD.
- Collect necessary documentation for specialty care referrals, including JHH TAP.
Communication:
- Participate in provider meetings and daily morning huddles, providing updates and discourse on SWOD’s interventions.
- Spend a minimum of one day per week in the medical pods for effective communication with the site medical team.
- Collaborate with medical providers for seamless care coordination.
- Develop and maintain positive working relationships with medical teams across sites and external medical partner entities.
- Directly communicate with the JHH TAP Program Coordinator and organize associated paperwork.
Teamwork:
- Orient staff and students as assigned by the Director.
- Oversee the orientation of new Social Work hires in conjunction with the Director.
- Monitor and arrange for adequate daily staffing, ensuring appropriate Social Work coverage.
- Manage site scheduling, tracking outages, late arrivals, and early dismissals on the weekly schedule.
Compliance, Policy, and Procedure:
- Document all services as directed by policy, state requirements, and professional standards.
- Adhere to departmental policies and procedures.
- Demonstrate appropriate use of billing, grant, or orders coding for all EHR documentation.
Patient Care:
- Refer patients to on- and off-site community resources.
- Provide clinic and triage coverage as assigned.
- Maintain an established caseload of patients, monitoring medical adherence and psychosocial barriers to care.
Skills and Abilities:
- Excellent verbal and written communication skills.
- Ability to work with diverse populations.
- Knowledge of community resources and Ryan White service deliverables.
- Strong attention to detail and organizational skills.
- Ability to communicate confidently with providers of various disciplines.
- Adaptability to changes within the organization and the evolving field of Health Care.
Education and/or Experience:
- Master’s Degree in Social Work.
- Active Maryland State Social Work License (LGSW, LCSW, LCSW-C).
- MHBE Insurance Assister/Counselor Certification within one year of hire.
- Four years of related experience in a health care setting, including community, case management, and/or outreach experience.
