Behavioral Health Integrated-Care Intake Coordinator

Elevation Health Services is currently seeking experienced behavioral health integrated care intake coordinators that have a passion for providing the highest quality care despite any barriers while creating partnerships with community agencies to create long term success for clients. In some areas these professionals are also referred to as case managers or intake clinicians. In our organization, these professionals are placed in the position of Behavioral Health Integrated Care Intake Coordinator to plan and provide integrated case management and develop an individualized treatment plan based on risk/needs assessment.

GENERAL DESCRIPTION OF RESPONSIBILITIES :

The BHICC will be responsible for utilizing an integrated case management model that includes assessment, planning, implementation, care coordination, and monitoring. Behavioral Health Integrated Care Intake Coordinator may serve various member populations such as GMH, criminal justice, substance use, etc.

MINIMUM QUALIFICATIONS :

• Master's degree required
• Minimum 3 years’ experience in Behavioral Health Care industry.
• One-year behavioral health experience working with adult justice-involved population
• One-year experience working in assessment and intake with behavioral health clients.
• Strong knowledge of wellness, recovery, and self-help
• Effective communication and written skills are essential
• Proficiency with computer and databases
• Knowledge of psychiatric diagnosis, psychotropic medications, various treatment interventions, and community resources

All new hires are expected to work in office everyday for a minimum of 30 days. Once they meet the standards of their probationary period then they will have the option to go hybrid/remote. This is not a PT or fully remote position.
