Job DescriptionPosition Title: Social Worker – Per Diem FLSA STATUS: Non-Exempt
REPORTS TO: Manager of Clinical Practice
SUPERVISES: N/A
DEPARTMENT: Home Health
POSITION SUMMARY :
The Social Worker – Per Diem’s primary role is to plan, organize, develop, and implement medically related social services in accordance with current federal, state, and Joint Commission standards, guidelines and regulations, the organization’s Health Center Administrator, to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.
This position is a Per Diem role.
PRINCIPLE DUTIES/ESSENTIAL FUNCTIONS :
The following duties are normal for this position. This list is not to be construed as exclusive or all inclusive. Other duties may be required and assigned.
 
  • Provide medically related social services to patients and families.
  • Assist in the development of department policies and procedures based on the current federal, state, and Medicare regulations.
  • Complete required documentation including admission notes, weekly/monthly progress notes, and IDG notes biweekly.
  • Participate in IDG meetings and attend regularly scheduled care plan meetings as required.
  • Coordinate family meetings as indicated.
  • Advocate on behalf of patients and families regarding issues as appropriate based on patient Rights, including review of patient grievances.
  • Be aware of Medicare and Medicaid, and other financial and community assistance programs available to the patient and provide education as indicated.
  • Serve on committees as required and as appointed by the Clinical Manager.
  • Provide guidance to team members regarding upholding patient and family rights.
  • Must comply with organizational vaccine mandates and maintain required licensure and certification
  • Provide medically related social services to the residents and families.
  • Assist in the development of department policies and procedures based on the current federal, state, and JCAHO regulations.
  • Complete required documentation including admission notes, quarterly progress notes, and interim notes as necessary.
  • Coordinate family meetings as indicated.
POSITION REQUIREMENTS:To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Educational Requirements and Experience:
  • Must possess a Master’s Degree from an approved school of Social Work
  • Must be licensed in MA as a LICSW or work under the direct supervision of an LICSW
  • Must have, as a minimum, one year experience as a social worker in a skilled nursing facility, hospice, hospital, or other related medical facility
  • Experience with geriatric, oncology and/or hospice preferred
  • Strong verbal, written and interpersonal communication skills
  • Possess valid State of Residence Driver’s license and reliable transportation
