Join our Team! We have an exciting new opportunity for an HCBS/CORE Specialist (Generalist) in our Home and Community Base Services in Tarrytown, NY. Base Hourly Rate Range is $22.00 to $24.00 per hour.

SUMMARY of DUTIES:  
HCBS provides opportunities for Medicaid beneficiaries with behavioral health conditions and substance use disorders (SUD) to receive services in their own home or community. HCBS utilizes a recovery model of care, which emphasizes and supports a person's potential for recovery by optimizing quality of life and reducing symptoms of behavioral health conditions and substance use disorders through empowerment, choice, treatment, educational, employment, housing, and health and well-being goals. Recovery is generally seen in this approach as a personal journey rather than a set outcome, and one that may involve developing hope, a secure base and sense of self, supportive relationships, self-direction, social inclusion, and coping skills. The Specialist will support recovery by providing services that reflect our deeply held values that include the recognition of individual rights of self-determination, choice, shared decision-making and collaboration. When possible he/she will utilize evidence-based practices; always embracing a trauma-informed and person-centered approach.

SPECIFIC DUTIES and RESPONSIBILITIES: 
1. The intent of HCBS programs is to restore the individual’s functional level to the fullest possible and as necessary for integration of the individual as an active and productive member of his or her family, community, and/or culture with the least amount of ongoing professional intervention.
2. Weekly schedule may include evenings and weekends. 
3. Other related duties. 
4. Assist individuals with a behavioral health diagnosis in acquiring, retaining and improving skills in the following areas: 
· Rehabilitation
o Psychosocial Rehabilitation (PSR) 
· Habilitation 
· Empowerment Services/Peer Supports 
· Support Services
o Family Support and Training 
· Individual Employment Support Services
o Prevocational Services 
o Intensive Supported Employment 
o On-Going Supported Employment 
· Educational Support Services 

REQUIRED KNOWLEDGE AND SKILLS: 
1. Prior knowledge and/or use of Trauma-Informed, Person-Centered Practice and Motivational Interviewing.
2. Knowledge of the Recovery process and the ability to facilitate recovery using established standardized mental health processes.
3. Knowledge and skill to teach and engage in basic problem solving strategies to support individual clients in self-directed recovery.
4. Knowledge of the common challenges associated with mental health conditions.
5. Ability to master an electronic record system, meet deadlines and willingness to grow and change are necessary.

QUALIFICATIONS:
1. The HCBS Specialist should have a Bachelor’s Degree in a human services field from an accredited college or a comparable combination of education and work experience, which should include at least 2 years of employment in the mental health/ social services field.
2. The HCBS Specialist may be a Certified Peer Specialist/Certified Recovery Peer Advocate, or equivalently qualified by education in the human services field or a combination of work experience and education.
3. The HCBS Specialist must be able to document services/supports according to agency and funder requirements. 
4. Computer proficiency is required.
5. Must be reliable and able to work independently and understand the importance of maintaining confidentiality.
6. The Specialist must have a valid Driver’s License and a driving record that is satisfactory to our insurance carrier.
7. If driving agency vehicles, the Specialist must maintain current New York State Defensive Driving certification.
8. The Specialist must be cleared by the NYS Justice Center through fingerprinting for a Criminal History Records search.
9. The Specialist must be cleared by the NYS Office of Children and Family Services for instances of child abuse and/or neglect.
10. The Specialist must be legally eligible to work in the United States.
