PURPOSE

To provide Service Coordination to participants referred to Workforce Services by the Pennsylvania Office of Vocational Rehabilitation (OVR). Implement the company mission and abide by all Matrix Policies and Guidelines and those identified by referral source to ensure that participants receive timely, efficient and quality services. Meet all specified performance outcomes and goals.

KNOWLEDGE AND SKILL REQUIREMENTS

Possess a solid understanding and knowledge of what employers are looking for. Understanding of the Americans with Disabilities Act, how to request an accommodation and what is required to work with individuals who have disabilities including basic disabilities knowledge. Be highly knowledgeable about the methods of job development and job placement. Be able to market participants in a way that would be appealing to employers and sensitive to the individual needs of the participant. Have good social and communication skills, patience, and time management. Demonstrate confidence, honesty and reliability with the ability to carry out professional code of ethics. Knowledge in the use of computers and basic office software (Word, Excel). Be goal oriented and highly organized.

ESSENTIAL JOB FUNCTIONS

The following is a summary of the essential functions of this job. The individual may perform other duties, both major and minor, that are not mentioned below. Essential functions may change from time to time.

Job Searching (Phase 1)

• Attend all weekly job search appointments with participant
• Provide at least 4-6 job leads per week using internet or whatever means necessary to provide appropriate and viable job leads. All job leads should fall within the participants abilities and functional levels.
• Provide Job development services per Pennsylvania Office of Vocational Rehabilitation Provider Manual guidelines.
• Provide follow up via e-mail or phone call to Vocational Rehabilitation Counselor to ensure that counselor is update and well informed about participants progress.
• Assure that the participant is compliant with the program and report to the VRC any significant issues

On the Job Supports (Phases 2-5)

• Contact or meet with participant on site or off site to provided need on the job supports
• Implement hands on and/or supportive guidance to ensure that the individual successfully retains position secured and learns position duties
• Assure that the participant is compliant with the program, employment requirements and report to the VRC any significant issues
• Comply with the standards set forth by Matrix Vocational Solutions
• Perform other related duties as assigned.

Youth Services (Pre-ETS)

• Attend all weekly appointments with participant
• Model professional behavior and teach employment-related soft skills
• Contact or meet with participant on site or off site to provided need on the job support
• Provide Pre-Employment Transition Services per Pennsylvania Office of Vocational Rehabilitation Provider Manual guidelines
• Provide follow up via e-mail or phone call to Vocational Rehabilitation Counselor to ensure that counselor is update and well informed about participant’s progress.
• Assure that the participant is compliant with the program and report to the VRC any significant issues

Case Documentation

• Complete legible well written reports on each case in accordance with requirements specified by the Pennsylvania Office of Vocational Rehabilitation in the Vocational Rehabilitation Provider Manual.
• Enter billings and outcomes information into Matrix systems as specified by Matrix Policy and Guidelines.
• Follow all documentation guidelines in compliance with CARF.

QUALIFICATIONS

The following qualifications are the minimum requirements necessary to perform the essential functions of the job.

1. Course work in social services, education, psychology, or social work preferred.

2. Knowledge of the Rehabilitation field, trends and methods.

3. Must have knowledge of disability.

4. Must have a valid driver’s license with reliable transportation and willing to travel.

5. Must have excellent communication skill and problems solving abilities.

Job Type: Contract

Pay: $18.00 - $20.00 per hour
