Responsibilities:

• Provide direct patient care, including assessment, planning, implementation, and evaluation of patient's condition.
• Evaluate patient needs in order to provide a personalized plan.
• Ensure patient's compliance with medication and care plans.
• Ensure the care our patients receive is coordinated, safe, timely, effective, efficient, equitable and patient centered.
• When needed, administer medications and treatments as prescribed by the physician.
• Maintain accurate and up-to-date patient records, ensuring compliance with HIPAA regulations.
• Collaborate with interdisciplinary healthcare team members to develop and implement patient care plans.
• Educate patients and their families about their medical conditions, treatment plans, and self-care techniques.
• Other duties as assigned.

- Must be able and willing to travel between fascilities on a daily basis.

Experience:

• Minimum of 2 years of experience as a Medical Case Manager
• Minimum of 2 years as a Registered Nurse in an acute care setting
• Proficient in using electronic medical record systems such as Epic
• Experience in home care or community health settings is a plus

Skills:

• Acute pain management techniques
• Strong understanding of HIPAA regulations and maintaining patient confidentiality
• Excellent communication skills to effectively interact with patients, families, and healthcare team members
• Excellent follow-up and organizational skills
• Triage skills to prioritize patient needs based on severity of condition
