Job Description: Registered Nurse MDS Coordinator (Skilled Nursing Facility)
Position Overview:
We are seeking a skilled and compassionate Registered Nurse MDS Coordinator to join our dedicated team at our reputable Skilled Nursing Facility. As an MDS Coordinator, you will play a crucial role in ensuring the accurate assessment and care planning for our residents in compliance with state and federal regulations. Your expertise and attention to detail will directly impact the well-being and quality of life of our residents.
Responsibilities:
Resident Assessment:
Perform comprehensive resident assessments using the Minimum Data Set (MDS) to determine their physical, emotional, and social needs.
Regularly review and update resident care plans to ensure they reflect the resident's evolving health status and individual preferences.
Collaborate with interdisciplinary team members, including physicians, therapists, and social workers, to ensure comprehensive care planning.
Medicare and Medicaid Compliance:
Stay up-to-date with federal and state regulations related to MDS and ensure compliance in all aspects of the assessment process.
Ensure accurate and timely completion of MDS assessments, transmission, and submission to regulatory bodies.
Collaborate with billing and coding staff to ensure proper reimbursement for Medicare and Medicaid services.
Quality Assurance and Improvement:
Participate in Quality Assurance and Performance Improvement (QAPI) initiatives to identify areas for improvement in resident care and services.
Analyze data from MDS assessments and use it to develop strategies for enhancing resident outcomes and overall facility performance.
Staff Training and Education:
Provide education and training to nursing and care staff on MDS assessment, documentation, and compliance standards.
Promote a culture of continuous learning and best practices to enhance the overall quality of care provided.
Interdisciplinary Communication:
Facilitate communication between different departments and care teams to ensure the smooth implementation of resident care plans.
Act as a liaison between the facility and external agencies, such as regulatory bodies and auditors, in matters related to MDS assessments.
Clinical Expertise and Support:
Demonstrate clinical expertise in geriatric nursing and act as a resource for the nursing staff, providing guidance on complex resident cases and challenging care situations.
Requirements:
Valid and unrestricted Registered Nurse (RN) license in the state.
Minimum of [X] years of experience as an RN, with experience in a skilled nursing facility preferred.
Certification as a Resident Assessment Coordinator-Certified (RAC-CT) is desirable but not mandatory.
In-depth knowledge of the Minimum Data Set (MDS) assessment process and regulatory requirements.
Strong understanding of Medicare and Medicaid guidelines and reimbursement principles.
Excellent communication, interpersonal, and organizational skills.
Proficient in electronic medical record systems and computer literacy.
Ability to work independently and as part of a team, demonstrating strong leadership qualities.
