Job Description

The Resource Nurse is responsible for providing clinical support and guidance to staff within the nursing home facility. This role involves assessing, planning, implementing, and evaluating nursing care plans for residents, as well as providing education and training to nursing staff to ensure compliance with regulatory standards and best practices. The Resource Nurse collaborates with interdisciplinary teams to promote resident well-being and safety while fostering a culture of excellence in nursing care.

Duties/Responsibilities:

  • Conduct comprehensive assessments of residents' health status and develop individualized care plans.
  • Provide direct nursing care as needed, including medication administration, wound care, and documentation.
  • Serve as a clinical resource and mentor to nursing staff, providing guidance on complex medical cases, procedures, and documentation.
  • Conduct regular audits and assessments to ensure compliance with regulatory standards and identify areas for improvement.
  • Develop and implement training programs for nursing staff on topics such as infection control, fall prevention, and resident rights.
  • Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to coordinate care plans and optimize resident outcomes.
  • Participate in quality improvement initiatives and assist in the development of policies and procedures to enhance the quality of care.
  • Act as a liaison between nursing staff, residents, families, and other healthcare providers to address concerns and facilitate communication.
  • Assists and maintains accurate and up-to-date medical records, including documentation of assessments, care plans, and interventions.
  • 1Assist with admitting and discharging patients and residents from the Health Center.
  • Monitors, tracks and trends infections, antibiotic usage. Responsible for assuring plans of care accurately represent the plan and communication related to each individual infectious incident. Assures staff are trained and compliant with appropriate PPE based on the postings, PPE set-ups are in place and stocked.
  • Antibiotic Stewardship reports are completed monthly based on the CMS guidelines and reports presented to the QAPI committee. Training and education will be provided to staff following the findings in each report.
  • Provide support and back up for the MDS coordinator.

Skills & Abilities:

Education

Valid nursing license (RN or LPN) in WA state.

Experience

Minimum of [5] years of nursing experience in a long-term care or similar setting.

Strong clinical assessment and critical thinking skills.

Excellent communication and interpersonal abilities.

Leadership qualities with the ability to motivate and empower others.

Proficiency in electronic medical records (EMR) and other healthcare technology.

Commitment to upholding ethical standards and promoting resident-centered care.

Willingness to work collaboratively in a dynamic and fast-paced environment.

Ability to prioritize tasks and manage time effectively.

CPR certification.

Other Requirements: Ability to communicate in English effectively with others.

Physical Requirements: 

- Ability to stand, walk, and move freely throughout the facility.

- Lift and transfer residents as needed, with assistance from mechanical devices or other staff.

- Perform tasks that require bending, stooping, and reaching.

- Work rotating shifts, including nights, weekends, and holidays, as required.
