The LPN will provide care for residents within Assisted Living and Memory Care and assist the Health and Wellness Nurse. The LPN will round all residents, assist the Health and Wellness Nurse by recognizing changes in condition, coordinating and drafting resident assessments for RN review and approval and handling incident reports and related investigations. The LPN provide ongoing staff development and attend training needed in order to be in compliance by state and federal laws.

Essential Functions Statements:

1. Draft resident assessments and Service Plan for RN review and approval.. Assessments and Service Plan must be updated at least annually as needed. 
2. Ensures training, orientations and in-service education is provided to staff in accordance with current Assisted Living Regulations. 
3. Assess physical, spiritual, and psychosocial needs of resident and coordinate with consultants as needed. Coordinate need for extras, such as home care, room service meals, MSW, etc.
4. Reports all incidents, investigates and notifies appropriate individuals.
5. Reports and investigates all incidents of abuse, suspected abuse, and negligence. 
6. Reviews MD orders/ MD notes, Therapy, Social Services, Mental Health notes for change in functions/ability.
7. Assures accurate count of all narcotics by shift.
8. Assures new orders are transcribed correctly and necessary medication is requested from pharmacy in timely manner. 
9. Will assist in other duties assigned by the Health and Wellness Nurse.

Skills & Abilities

Education: Graduate of LPN Nursing Program

Experience: 1 year of nursing / Prefer experience in Assisted Living

Computer Skills: Ability to effectively utilize basic software and email 

Certificates & Licenses: Current Washington State LPN License

Other Requirements: Current Basic Life Support (BLS)
