We have an exciting opportunity for a Residential Case Manager to join our team of talented professionals.

As a Residential Case Manager in the Adult Behavioral Health Program, you will be responsible for the following:

  • Provide case management services to adult with mental illness who are placed in or in the process of transition to or from residential settings including specialized adult foster care, semi-independent living and transitional residential facilities.
  • Provide a “welcoming” experience to clients and families and to conduct thorough and comprehensive case management assessments with adult mental health clients in an effort to accurately identify their service needs and preferences.
  • Must maintain an acute awareness of services, therapeutic activities and supports available within and outside of the agency that may be of benefit to individual clients and their families.
  • Responsible to help ensure access to services, benefits and entitlements, monitor and coordinate the delivery of services with other treatment team members, service providers and community organizations.
  • Conducts skill building interventions with individuals and groups of consumers to help them learn to access services independently, and make more effective use of community resources available to them.
  • Advocacy efforts include community outreach and transportation assistance as needed.
  • Regular travel to residential settings is required.
  • Services are delivered primarily in the community. Regular working hours are Monday through Thursday 8:30 a.m. to 7:00 pm, Friday 8:30am to 5pm and Saturday 9am to 1pm. Flexible scheduling is available. Some evening or weekend hours will be required based on program needs.

Requirements

To qualify for this position, you will need the following:

  • Candidates must be a human services professional with at least a bachelor's degree in a human services field.
  • Candidates must obtain the Qualified Mental Health Professional credential within 120 days of hire.
  • Must have one year of experience (may include time spent in fieldwork or internship), treating or working with people who have been diagnosed with mental illness.
  • Must possess a valid driver’s license and a good driving record. Must obtain a Michigan Chauffeur License within 30 days of employment.
  • Also must either: (1) possess a CADC/CAADC credential, or (2) have proof of application and payment submitted to the MCBAP for CADC/CAADC, or (3) submit the MCBAP Professional Development Plan within 30 days of hire. Approvals of plans or credentialing must be completed within 90 days of hire for continued employment.
  • Must have the ability to engage adults with mental illness and/or co-occurring substance use disorders, their guardians, families and supports.
  • Candidate must be an expert in helping clients identify their service needs and preferences, and helping them build on their individual strengths, resources and supports to navigate the network of services available at the agency and in the community.
  • Must have an orientation toward providing community outreach services. Additional requirements include a nonjudgmental approach that reflects cultural competency and sensitivity to the unique needs and diverse experiences of adults with mental illness, their families and providers.
  • Candidates must be able to work independently, demonstrate effective organizational, verbal and written communication skills.
