Minimum Qualifications: BA in Social Services and 6 months of experience, or 2 years’ experience as a case manager. Proficiency in Microsoft Excel, Word, and Forms is required. Experience in Salesforce a PLUS!
The Community Connection Specialist is integral in helping The Family Center pursue our mission of breaking intergenerational cycles of childhood trauma through the implementation of service coordination for the Family Resilience Department. This position requires forward thinking, ability to be proactive, and a strength-based and trauma- informed approach to the role, including embracing curiosity to learn and grow personally and professionally, greeting challenges with flexibility and a collaborative approach, and pursuing excellence in work products and community presence.
JOB OVERVIEW:
This position reports directly to the VP, Family Resilience and for the “day-to-day” needs reports to the Manager, Family Resilience Programs. As a member of the program team, the Community Connection Specialist is responsible for assisting adults, youth, and families in determining their needs, developing, and coordinating the case plan, linking with needed housing, medical, educational, social, emotional, and vocational services in the community, and acting as a liaison between all support services being provided. In addition, provide support gathering intakes and assessments, ensuring proper follow up and documentation in a timely manner, and additional needs as assigned. The Community Connection Specialist is expected to embrace and demonstrate core organizational values and adapt to diversity and the cultural contexts of individuals and communities served. They are expected to regularly attend all meetings. Does not supervise any other staff or volunteer.
ESSENTIAL DUTIES & RESPONSIBILITIES:
- Demonstrates knowledge of and acts according to Child and Elder Abuse Reporting laws.
- Develop appropriate relationships and maintain appropriate boundaries with assigned participants, youth, and families.
- Engage participants and family in accessing program services, housing, education, and any community resources needed.
- Coordinate referrals and linkages to identified resources in the community, including follow-up.
- Administer designated Needs Assessment Tool to increase client engagement, assist client to identify needs and progress towards meeting goals.
- Provide job readiness training curriculum that includes resume writing, interviewing skills, job search strategies and application process support.
- Develop and maintain collaborative partnerships with public and private community agencies to facilitate referral and linkage opportunities for participants.
- Liaison with community resources to develop and maintain resource library.
- Coordinate transportation for client as needed to meet identified goals.
- Complete and submit timely documentation on each required service according to contract and/or program requirements.
- Provides accurate and timely status updates according to identified protocols.
- Maintains a variety of confidential and non-confidential records for the purpose of data entry, audit trails, and/or ensuring the availability of data.
- Participate in ongoing staff development and training; including, but not limited to: ACES’s, strength-based, cultural competency, and trauma informed approaches to service delivery.
- Ensure all program activities operate within TFC’s policies including sensitivity and inclusiveness regarding cultural, religious, ethnic, gender and sexual diversity, and comply with relevant legislation and professional standards.
- Other duties as assigned.
QUALIFICATIONS:
- BA in Social Services and 6 months of experience or 2 years’ equivalent experience in social services and/or as a case manager. Bilingual *preferred.
- Previous experience working with individuals who are incarcerated or in reentry preferred*
- Ability to respectfully engage and support participants within the community and correctional settings required.
- Have working knowledge about mental health and substance abuse.
- Have a working knowledge of local and state community resources.
- Ability to effectively interact, communicate, and collaborate with people from diverse racial, cultural, religious, or gender identifying backgrounds.Demonstrate cultural and linguistic competencies, value diversity, and an ability for self-awareness.
- Ability to develop respectful and supportive relationships with key partners, stakeholders, and program participants.
- Ability to function in a team environment and interact productively with various organizational cultures and work independently.
- Solid writing skills to satisfy project documentation requirements.
- Flexible and non-structured work hours.
- Proficiency in Microsoft Excel, Word, and Forms is required.
- Experience working with Microsoft Office and program databases (Salesforce preferred).
- Successful background and criminal clearance required and maintained.
Proof of a valid Driver’s License, reliable transportation, and proof of automobile insurance
This is a full-time exempt position that requires evening and weekend commitments throughout the year. Salary and benefits are commensurate with experience and organizational budget.
