CSN Kansas City is currently offering a $1500 Hiring Bonus for candidates who apply through 7/1/2024 and are successfully hired. The hiring bonus is paid in 3 installments: $200 - First paycheck following completion of initial two-week training, $500 - 3 months of employment and all training complete, $800 - 6 months of employment. *Applicants previously employed by CSN within the past 6 months are not eligible for this bonus.

Job description

As a Care Coordinator, you will develop and lead team members to deliver quality services that help support individuals to learn, grow, and live their best possible lives and thrive as members of their community! This is a salaried position requiring evening and weekend availability.

• Assist with medication management
• Schedule ALL doctor Appointments
• Accompany supported individuals to ALL doctor appointments
• Provide habilitation & documentation
• Ensure all documentation is uploaded into Therap
• Support individuals to practice and maintain good hygiene
• Follow health & safety protocols
• Engage and coach Direct Support Professionals
• Additional duties assigned per company needs

What You’ll need

• 1 Year experience in field of intellectual disabilities and/or behavioral services
• Must be 18 years of age or older
• Valid driver’s license and current auto insurance
• Reliable vehicle to transport during shift
• Pass all background checks
• Full range of motion, physical strength, and the ability to achieve and maintain training and certifications
• Proficient with Microsoft Office
• Bachelor’s degree in social work, psychology or a related field with at least two years of increasingly responsible supervisory or management experience in a health care field. Must also possess a valid driver’s license and auto insurance and have reliable transportation. Must receive favorable results from background and reference checks.
