About us

The Collaborative for Children and Families (CCF) is a coalition of more than 25 community-based member organizations. Our member organizations have decades of collective experience working with children and families and are deeply rooted within the communities we serve. CCF partners with Hospitals, Managed Care Organizations (MCOs) and community-based healthcare providers—serving all five boroughs in New York City, Westchester, Nassau and Suffolk counties.

CCF is a Health Home Serving Children and Families as well as a designated Children's Home and Community Based Services (HCBS) provider. We are designated by New York State Department of Health striving to keep children and families in NYS healthy!

Through CCFs network partnership we have created a seamless way to access providers that can conduct physicals, perform complex mental health assessments, and provide treatment to those who are Medicaid eligible. We work together to improve access to healthcare services for children and their families which will result in better health outcomes and an improved quality of life by providing best quality care coordination and community based services to achieve more efficient and better outcomes for all.

Position Summary and Responsibilities: The Planned Respite Service Provider, works under the direct supervision of the Community Based Services Provider Supervisor and provides community-based services that focus on short-term assistance provided to children/youth, regardless of disability, because of the absence of or need for relief of the child, or the child’s family caregiver. Respite workers supervise the child/youth and engage the child/youth in activities that support his/her and/or primary caregiver/family’s constructive interests and abilities. Respite Service Providers will offer services with a level of expertise in understanding and implementing behavioral/developmental interventions required to support optimal functioning for children/youth during planned care.

Essential Duties and Responsibilities

  • Planned Respite (PR):Provide short-term relief for the child or family/primary caregivers that are needed to enhance the family/primary caregiver’s ability to support the child/youth’s functional, developmental, behavioral health, and/or health care needs.

Planned Respite activities support the plan of care goals and include providing supervision and activities that match the child/youth’s developmental stage and continue to maintain the child/youth’s health and safety.

  • Development and updating of Person Center service plans (initial plan within 30 days of first service and every 6 months thereafter) including obtaining medical consenter signatures at the initial session following the approval of such documentation.
  • Obtaining consents and enrollment documents for newly enrolled children/youth at initial session and scanning documents into the Electronic Health Record.
  • Attend monthly waiver service provider meetings and individual supervision to stay current on practices and protocols.
  • Submit progress notes, service plans, and service provider logs within set forth-required timeframes. It is the responsibility of the provider to maintain necessary technology (phone, internet, timely computer access, and video camera) to perform job requirement functions of documentation, coordinating, service planning and billing.
  • Report incident(s) to supervisor upon discovery.
  • Maintain open communication with other service providers (e.g., Care Manager, other service providers and Supervisory Team) to report on child/youth’s progress, and identify any potential barriers and referrals, and to support the overall health and wellbeing of the child and family.
  • Check emails daily and respond to emails from Care Managers, Supervisors, and other team members within 24-48 hours.
  • Setting of Service Provision:Planned Respite can be provided in the home of the youth or a community setting.

Essential Skills, Experience, Licensure, Certification:

Qualifications:

  • Respite providers are minimally required to have a high school diploma or equivalent and with appropriate skills and training.
  • Experience working with children/youth.

  • Maintains a strength-based approach in working with children and families.
  • Accepts and applies constructive feedback from supervisors to enhance the quality-of-service delivery.
  • Be a team player.
  • Have a strong sensitivity to cultural differences present among staff and members within our organization.
  • Possess a strong belief in people’s ability to grow and change; forge a mutually respectful partnership with persons served and their families.
  • Ability to set limits and maintain helping role of practitioner and to intervene appropriately.
