MENTAL HEALTH PROFESSIONAL (MHP)

Skokie Meadows is seeking a Mental Health Professional (MHP)

Responsibilities

  • Demonstrating the mission and vision of Breakthrough, the Mental Health Professional is responsible for mental health assessments, writing biopsychosocial histories, and providing individual counseling and care coordination for guests experiencing homelessness residing at Breakthrough Urban Ministries Women’s Center
  • The MHP works with other members of the Adult Services team to assist guests with their mental health needs, including co-occurring substance use disorders, as guests work toward stability, which can include sobriety, employment, and permanent housing
  • Provide mental health counseling to guests as needed, specifically those with co-occurring diagnoses that include complex post traumatic and substance use disorders
  • Engage in comprehensive care coordination to link guests to community-based behavioral health programs/agencies for long-term mental health, medical and substance use services
  • Provide crisis management, including involuntary commitment when needed to gain stabilization
  • Collaborate with multidisciplinary team and external referral sources
  • Facilitate weekly trauma recovery and substance use recovery groups
  • Enter required information into agency database
  • Submit written monthly reports to the Behavior Health Director
  • Assist with completion of pending benefit claims, including SSI, SSDI, etc

Qualifications

Bachelor degree in Social Work or Counseling; Experience working with homeless or disenfranchised clients and an understanding of behavioral health issues common to the homeless population

Understanding of standard mental health assessments and counseling theories, skills, and trauma-informed practice

Job Type: Full-time 3-11pm or 4-10pm, Part-time or Weekend shift
