The Childrens Community Programs of Connecticut is Hiring a Case Manager-Bilingual Near New Haven, CT

Summary:

A case manager is responsible for managing the day-to-day activities of UC foster youths placed in CCP transitional foster homes and ensuring that their clients’ needs are being met.

Essential Functions:

  • Ensure the safety of long term foster care clients.
  • Ensure the needs of the UC are being met, including but not limited to the physical, medical, emotional, behavioral, cultural, social, and educational needs of the child.
  • Responsible for care planning, including, but not limited to service, safety, crisis, and discharge planning.
  • Coordinate services with all other providers.
  • Collaborate with the UA, sponsor family, foster family, Office of Refugee Resettlement (ORR), and all service providers regarding the clients’ safe and timely release.
  • Provide foster family support, guidance, education, and supervision.
  • Provide client transportation when necessary.
  • Maintain all appropriate service documentation, including progress notes, case management notes, and service plans
  • Maintain all appropriate ORR and program paperwork

Supervisory Responsibility:

  • None.

Required Education and Experience:

  • Bachelor’s degree and at least 3 years of experience providing services to children and/or families, preferably in the child welfare system.
  • Bilingual English/Spanish

Preferred Education and Experience:

  • Master’s degree in the behavioral sciences, human services, or social services fields
  • Previous experience in providing services to immigrant populations, particularly unaccompanied minors or minor refugees.

Job Type: Full-time

Pay: $52,000.00 - $55,000.00 per year
