The Center for Great Expectations is Hiring a Clinical Case Manager Near Passaic, NJ

Job Description

Clinical Case Manager, START Passaic

Will consider master’s level students who are graduating in May 2024 and applying for State licensure (LAC, LSW and/or LCADC) within the next six months.

Under the supervision of the Site Supervisor, the Clinical Case Manager will provide case management and clinical services to START clients seeking treatment for substance use disorders and/or mental health disorders to support and strengthen their capacity to engage in healthy practices. Also, the Clinical Case Manager will operate as a member of a larger treatment team in planning and providing program services. The Clinical Case Manager will promote the use of the Nurtured Heart Approach, as well as the Attachment, Regulation, and Competency (ARC) model for trauma-informed care in all activities with the goal of sustaining a safe and comfortable environment for the clients served within the program. Local travel is required.

1.) Perform all duties and demonstrate behaviors and attitudes consistent with the Mission Statement and Core Values of the organization, and the Nurtured Heart Approach.

2.) Continually participate in building and maintaining positive working relationships through effective communication, performance improvement and teamwork efforts.

3.) Be of good character and reputation; sufficient physical, mental, and emotional health to satisfactorily perform her or his job duties.

4.) Actively participate in the organization's safety programs by identifying potential risks and promoting client safety and environmental safety.

5.) Comply with HIPAA guidelines regarding client rights and confidentiality.

6.) Comply with documentation and training set forth by Human Resources

  • Regularly assess client needs with the team and ensure proper case management services are delivered.
  • Provide screening, brief intervention, and provide appropriate resources, services, and/or opportunities.
  • Work with clients to ensure direct case management services are delivered and/or proper referrals are provided to clients for services including but not limited to; educational and vocational pursuits, legal issues, recovery support, life skills (e.g. resume writing, budgeting, etc.), and accessing housing, social services and other community resources through to completion.
  • Assist team with outreach services to clients in the community and in their homes. Actively engage clients weekly or as needed. Provide transportation to clients and their children as needed.
  • Advocate on behalf of the client and/or their family.
  • Work within team framework to maintain ongoing relationships with various community agencies/providers and act as a liaison to human service agencies that meet identified client needs.
  • Participate in program and other CGE meetings, family treatment team meetings and training program(s) as indicated.
  • Provide support to ensure seamless transitions among CGE’s continuum of care as needed.
  • Provide proper crisis management/intervention as needed and notify supervisor as soon as practical.
  • Exercise independent judgment when observing and assessing a client’s current functioning.
  • Respond to clients in an attuned and effective manner, employing CGE’s protocols and procedures as appropriate to presenting situation.
  • Make appropriate judgments with respect to life threatening circumstances and take appropriate action.
  • As needed, assist birth parents and resource parents create Lifebook containing routines and rituals to create consistency for the child(ren) during times of transition
  • Prepare and deliver oral and written reports to staff and appropriate agencies as indicated.
  • Employ ERH and SUD recovery strategies to clients and children dyadically when assigned to do so.
  • Complete clinical assessments and treatment plans, conduct individual sessions and discharge planning meetings as assigned.
  • Build a network of support within the program and among clients that focuses on trust, well-being and social/community integration.
  • Continually participate in building and maintaining positive working relationships through effective communication, performance improvement and teamwork efforts.
  • Work as a member of a team to establish and maintain a high level of care and respect for, and communication with clients.
  • Always work in a consistently professional manner, which includes, but is not limited to, treating all residents, staff, guests, and volunteers with dignity and respect.
  • Perform other duties as requested.
  • Master’s degree in human services, social work, psychology, counseling, education, health, or another related field.
  • LAC or LSW required. LCADC a plus.
  • Experience working with substance use and co-occurring disorders.
  • Experience working with clients in a home-setting experience preferred.
  • Extensive case management and community outreach experience.
  • Bilingual (English/Spanish) speaking a plus.
  • Solid written documentation skills.
  • Excellent organizational and interpersonal skills with friendly and professional demeanor.
  • Current NJ driver's license needed; will be required to meet safe driving requirements.
  • Ability to be flexible to the daily changing needs within the community and to handle obstacles with compassion and resolve.

Job Type: Full-time

Pay: $55,000.00 - $65,000.00 per year
