Field RN Case ManagerHealthCare Support is actively seeking a Field RN Case Manager to fill an opening with a Fortune top 50 Healthcare Organization in New Jersey (Passaic and Union Counties).Daily Responsibilities for Field RN Case Manager:
- Assist the provider/team with various care coordination activities
- Assist Providers in assuring complete and accurate documentation and coding, medication reconciliation, nursing rounds, assessment, and patient documentation
- Assist in managing members through an acute medical situation or an exacerbation of a chronic condition, including testing, diagnostics, interventions
- This position required travel between facilities in Passaic and Union counties.
- Monday – Friday 8:30am-5:00pm EST
- Associate degree or higher in Nursing (RN)
- Active and unrestricted license in New Jersey
- Certified in Basic Life Support
- Minimum 1 year working in a hospital or Long-Term Care facility as an RN
- Full COVID-19 vaccination is an essential requirement of this role
