The Center for Family Development is Hiring a Family Resource Specialist Near Pleasant View, TN

The Family Resource Specialist is responsible for the screening and assessment of parents in the prenatal or postnatal period for intensive home visitation services in accordance with the critical elements of Healthy Families America. The Family Resource Specialist is to refer families in need of extra support services to appropriate community programs or agencies. The Family Resource Specialist must promote good public relations and assist with community outreach activities. The Family Resource Specialist applies and implements the philosophy and mission of TCFD. Positions will be available serving Robertson, Sumner and Trousdale counties.

Duties

Screens hospital records in accordance with hospital policies when hospital screens are permitted.

Conducts Parent Survey’s (assessments) in hospital and homes of potential participants to determine appropriateness for home visiting program and/or other services.

Documents all service contacts with family using appropriate forms in case record in a timely manner according to program requirements.

Determines eligibility for home visitation services.

Meets with Program Director or Supervisor on a weekly basis to assure quality of outreach, assessments and other duties. On-going supervision occurs routinely when assessments are completed.

Meets with Family Support Specialists and their supervisor as new cases are assigned.

If family does not enter the program, makes referrals to appropriate community based agencies depending on family needs.

Performs community outreach, including obtaining screens on a routine basis from referral sites.

Performs periodic community outreach at local health fairs, school functions, and other appropriate events.

Completes relevant paperwork for record keeping, program data management system.

Initiates projects such as organizing resources and other activities to benefit program services for families.

Attends and helps in preparation (as required) of family support groups.

Attends and/or participate in conferences, workshops and other training related to job functions.

Uses own vehicle for travel to appointments.

Ability to work occasional evening and /or weekends as needed.

Qualifications

Bachelor’s Degree in Human Services or related field.

Experience in working with or providing services to children and families.

An ability to establish trusting relationships.

Acceptance of individual differences.

Experience and wiliness to work with culturally diverse populations that are present among the Healthy Families Program target population.

Experience in working with community agencies, including implementing referral processes.

Strong interpersonal skills/ability to relate to people with respect for their individuality.

Task oriented; behavior supports beginning and completing a project/task within a specific time frame.

Must have valid driver’s license, automobile for fulltime business use and proof of automobile insurance.

Good writing and organization skills.

Experience with Microsoft Office.
