Location: REMOTE (Monroe, LA)
Job Description
GENERAL DESCRIPTION:
The Center for Children and Families Therapeutic Services Assessment Therapist is responsible for assessing and completing all assessments and intake paperwork. The Assessment Therapist is supervised by the Crisis Intervention and Assessment Coordinator in Shreveport.
This is a contract position.
QUALIFICATIONS:
The Assessment Therapist should have the following skills and experience:
  • M.A. in Marriage and Family Therapy, Social Work or equivalent degree preferred; experience with families and children preferred.
  • Must have obtained LMSW, LMFT or LPC License
  • Willingness to provide hands-on, in-home concrete services.
  • Must have respect for cultural differences and families' rights to self-determination.
  • Must be able to assess situations quickly, and have sufficient ingenuity to cope with limited resources.
  • Ability to work well in a team setting.
  • The ability to adjust to various types of tasks and work cooperatively with different types of personalities.
  • Ability to prioritize multiple tasks
  • High standard of personal integrity and work ethic.
  • Thorough understanding of all treatment and other contract programs and an intimate knowledge of program procedures and policies.
  • The ability to build and maintain relationships with outside agencies.
  • The ability to communicate well both verbally and in writing.
  • Commitment to The Center for Children and Families and Therapeutic Services Division goals and mission.
ACCOUNTABILITY:
The Assessment Therapist is hired by and reports directly to the Assessment and Intake Coordinator in Monroe
RESPONSIBILITIES:
Assessment is the systematic evaluation of an individuals behavior(s), strengths, and needs through observations and descriptions. The assessment is used as a tool to help the clinician gain a general perspective of the problem and for clinical case formulation, which includes identification of targeted behaviors, development of treatment goals, and assist with the proper identification of therapeutic program.
Components:
  • Responsible for assessing and completing all assessments and intake paperwork in accordance with program, agency, and state requirements.
  • Assessment is completed via telehealth or face-to-face, and can occur in a variety of locations, including where the person lives, works, attends school and/or socializes.
  • Thorough understanding of all intake documents, all treatment programs offered within Therapeutic Services, general understanding of each programs service criteria, and knowledgeable of Intake and Program policies and procedures.
  • Strong engagement skills and ability to adjust to various types of tasks.
  • Ability to communicate well both verbally and in writing.
Supervision:
All therapists must attend supervision in accordance with program, agency, and state requirements. Assessment Therapist must attend weekly staffing with Program Coordinator to review all active assessments.
  • Contact with family and referent must be made within 24 hours (via e-mail, text, or call) once referral is received from Program Coordinator. If contact is not made within the 24-hour period then calling and attempted face-to-face contact is required.
  • Meet with family to complete all necessary Intake paperwork and Assessment paperwork through face-to-face means or through Telehealth. Submission of all Intake and Assessment paperwork is due within 48 hours.
Documentation:
Assessment Therapist are responsible for maintaining accurate and up-to-date case records on each client served. Documentation should be in accordance with program, agency, and state standards. Documentation of service may include but is not limited to:
  • Clinical Assessment, Preliminary Treatment Plans, and CALOCUS or LOCUS
  • Intake and Initial Service Forms
  • Confidentiality/ Client Rights/ HIPAA Forms and Related Documents
  • Generate and maintain case notes
  • Other Documents/Forms/Letters as required during the course of treatment.
Contract position, paid at a rate of $70 per assessment completed.
Minimum of 12 assessments completed per week.
