Each year, the Carroll Center for the Blind helps nearly 1,300 blind or visually impaired children, adults and senior citizens gain the skills they need to live active and fulfilling lives. We welcome individuals of all ages who are blind or visually impaired. Our Newton campus houses our rehabilitation program, computer training classes, low vision store, and administrative offices. A large portion of our programs are services conducted in public schools, students’ homes, and their communities.

We are looking for a positive, compassionate, and a care-focused medical professional to provide support to our clients as they develop new daily living skills and adjust to blindness or visual impairments. SUMMARY

The Nurse Healthcare Coordinator will foster a safe and encouraging environment to promote physical, emotional and overall well-being to a wide variety of clients. They will serve as an advocate and support to the clients, treating them and their families with compassion and respect and be a liaison with parents/guardians, counselors, and other external agencies. This individual will manage the special healthcare needs of clients, perform basic medical treatment as needed, and ensure the clients receive the essential education and instruction in how to manage their specific health, medical and dietary need(s), using adaptive techniques. Additionally, they will support and oversee staff training that is tailored for specific client needs, review health information, maintain accurate records and logs, monitor the clients well-being, and train staff about basic illness/injury types and related action steps.

RESPONSIBILITIES

  • Assess and review incoming clients’ health needs for successful participation in Carroll Center programs.
  • Participate in multi-disciplinary team meetings to report on health matters and provide guidance to the team and oversee training regarding clients’ specific health/dietary needs.
  • Provide instruction in adaptive methods to manage medication labeling/identification, managing diabetes and self-administration of insulin and checking of glucose levels, and self-administering wound care.
  • Teach group classes on health topics such as basic first aid, nutrition, exercise, and communicating with medical providers.
  • Instruct, communicate and act as liaison between clients, team members, families, physicians, hospital personnel, community agencies and other external stakeholders when indicated.
  • Empower and support clients to monitor any illness, the importance of engaging in medical appointments and their health while in our programs and how to access appropriate care.
  • Maintain client records and prepare necessary reports in a timely manner, in accordance with HIPAA.
  • Perform other related duties as assigned.

QUALIFICATIONS

  • Current RN, LPN or EMT license required.
  • 1-2 years of related experience required; experience in a rehabilitation or school setting and with client’s who have physical or developmental disabilities preferred.
  • Experience supporting clients with diabetes, brain injury and/or substance use disorders preferred.
  • Demonstrated working knowledge of and compliance with Department of Health (DPH) regulations.
  • Possess exceptional communication, interpersonal, and assessment skills required.
  • Demonstrated ability to be highly organized, detail oriented and of Microsoft Office Suite skills required.
  • Current CPR and First Aid certifications required.
  • Experience maintaining health forms, paperwork, and documentation.

Pay: $36.00 - $41.00 per hour
