Position Overview:

Responsibilities:

• Conduct in-home patient assessments and develop individualized care plans.
• Administer medications and treatments as prescribed by the healthcare team.
• Monitor and report changes in patient conditions, collaborating with the supervisory team.
• Provide education and support to patients and their families, empowering them to manage their health needs.
• Maintain accurate and up-to-date patient records in compliance with regulations.

Qualifications:

• Valid LPN/RN license in the state of New Mexico.
• Compassionate nature and dedication to improving patients' quality of life.
• Excellent communication and interpersonal skills.
• Ability to work independently and collaboratively within a multidisciplinary team.
• Strong organizational skills and attention to detail.
• Previous experience in home health or a similar setting is a plus.
