As a Case Manager within our Home Health team, you will play a pivotal role in delivering exceptional, specialized patient care to our home-bound clientele. Conducting comprehensive home visits as assigned, you will assess patient conditions, oversee progress toward established goals, and deliver personalized patient care, education, and advanced interventions as prescribed. Ensuring strict adherence to the State Nurse Practice Act, agency protocols, and recognized professional standards is of utmost importance. Drawing upon your extensive knowledge of nursing theory and expertise in the nursing process, you will effectively communicate and meticulously document observations and assessments.
Key Responsibilities:
- Tailor client education to individual requirements, considering the client's surroundings, personal values, and physical/emotional condition.
- Administer medications in accordance with prescribed protocols.
- Promptly communicate significant changes in client status to the physician and interdisciplinary team, aligning with client needs.
- Maintain meticulous client records, documenting assessments, planning, interventions, and evaluations systematically.
Requirements:
- Current licensure as a Registered Nurse in NM.
- Valid CPR certification.
- Prior experience in Homecare/Hospice settings is preferred.
