Aesthetic and Medical Practice in Northwest Houston is looking for a highly skilled individual to fill a position as Patient Care Coordinator.

The Patient Care Coordinator plays a pivotal role in enhancing the patient experience within our healthcare facility. As a liaison between patients and the care delivery team, this role involves managing patient care logistics, facilitating communication, and ensuring the seamless coordination of services to support patient needs effectively and efficiently.

· Patient screening

· Scheduling

· Communicate with nurses and physician

· Meet with patients to inform and educate

· Handle inbound calls and answer patient questions

· Coordinate with insurance carriers

· Process pharmacy orders

· Review medication inventory

· Maintain files

· Patient case management tasks

· Assist the care team with goal setting

· Help with data collection

· Maintain patient confidentiality

Experience and Education Required

· Strong communication skills

· Compassionate nature

· Above average critical thinking skills

· One year or more working in a healthcare environment

· Customer service experience in a medical setting, preferred

· English and Spanish
