Description

Texas Diabetes & Endocrinology, P.A. (TD&E) has continued to grow over the past 20 years to meet the Endocrinology needs of the Austin Community. Our well-established, busy clinical practice includes Board Certified Endocrinologists and a team withadvancedtraining, educational degrees, and professional experience. Our mission is to strive to provide expert care in Diabetes, Thyroid problems, Osteoporosis, weight management, and hormonal conditions by providing thorough evaluations and innovative, personalized solutions focused on our patient’s success.Our employees are the strength that keeps us moving forward.

We are seeking a full-time Advanced Practice Provider (Family Nurse Practitioner, Clinical Nurse Specialist, or Physician Assistant) to join the team in this multidisciplinary environment.

This candidate will work in our Central Austin clinic.

Candidates should be dedicated to consistently providing high quality, compassionate patient care. This candidate will work collaboratively with Endocrinologists, four other Advanced Practice Providers (two are certified diabetes educators), a Lipidologist and a tenured support staff. Our office hours are 8:00am-5:00pm, Monday-Friday. We are not open on weekends and our APP’s are not on call after hours.

Advanced Practice Provider Essential Functions:

  • Assesses the physical and mental condition of patients by performing and obtaining comprehensive or problem-focused physical examinations (i.e. Diabetic foot and eye exams), screenings (i.e. Nephropathy, Tobacco use) and medical histories.
  • Actively demonstrates the organization’s mission and core values and conducts oneself at all times in a manner consistent with these values.
  • Utilizes motivational interviewing techniques to help patients make behavioral changes and achieve personal goals.
  • Orders, collects, interprets, and evaluates laboratory and other diagnostic tests to assess patient problems and health care needs.
  • Prescribes, recommends and/or employs medical drug therapies or other forms of related therapeutic treatments and procedures.
  • Completes simple and complex procedures independently in accordance with approved protocols/policies with attention to efficacy, safety, and cost. Recommends interventions to modify behavior associated with health risks.
  • Formulates and implements comprehensive outcomes-based patient treatment plans based on patient assessments, evidence-based medicine, standards of care, professional practice guidelines and in collaboration with physicians and other interdisciplinary team members when appropriate. Evaluates patient’s response to health care provided and the effectiveness of care.
  • Counsels patients and families regarding medical processes, management of the health illness/condition, medication, nutrition, and health promotion. Counsels patients about drug regimens and possible side effects or interactions with other substances such as food supplements, over the counter medications and herbal remedies.
  • Maintains appropriate records detailing the patient’s treatment plans and outcomes during or within 24 hours of the encounter. Ensures that medical record documentation supports the level of service reported to a payer and meets medical necessity requirements.
  • Initiates timely consultation and referral when the problem exceeds scope of practice and/or expertise.
  • Adheres to medical knowledge and practice-based learning and improvement expectations. Completes professional development activities and maintains professional affiliations and annual Continuing Medical Education (CME's) on an annual basis.
  • Adheres to all local, state, and federal laws, regulations, and statutes. Maintains and follows standard quality, safety, environmental, and infection control policies and procedures.
  • Participates in medical record, patient care, quality assurance, process improvement and compliance audits, evaluations, and follow-up.
  • Attends and participates in mandatory meetings & training sessions as required.
  • Additional duties or projects as assigned.

Additional Non-Essential Functions:

  • Acts as a Sub-Investigator for Clinical Research Trials.
  • May be asked to provide precept training to physician extender students.

Qualifications


Advanced Practice Provider Qualifications & Experience:

  • Minimum two (2) years of hands-on experience as an Advanced Practice Provider
  • Recent experience in family medicine, internal medicine, and/or endocrinology is vital to your success.
  • Must love building relationships with patients, value bedside manner, and possess excellent clinical skills
  • Good verbal and written communication skills
  • Excellent interpersonal skills and must be able to be sensitive to cultural and bilingual issues
  • Critical thinking and complex problem-solving skills
  • Requires reasoning ability and good independent judgment.
  • Organizational skills are necessary
  • Skill in the use of computers and related software applications
  • State licensure and board certification is required
  • Bilingual a plus
