Registered Nurse (RN) *MORRIS COUNTY - FULL-TIME AND PART-TIME, DDD-APPROVED LICENSED GROUP HOMES*

QUALIFICATIONS:

  • Graduation from an accredited school of licensed practical nursing
  • State of NJ Board of Nursing Certification as a Licensed Practical Nurse
  • LPN or RN must have a current license to practice in the State of NJ
  • At least 21 years of age (Insurance Purposes)
  • Valid driver’s license with 4 points or less (Cannot be provisional/probationary)
  • 2 years working with medically involved adults with disabilities (hospital, sub-acute, assisted living, rehab/nursing facility, other medical facility, or a medical group home)
  • Successful completion of criminal background history and fingerprint examination

Required Skills and Abilities:

  • Experience in the supervision of those providing nursing care
  • Knowledgeable about specific state laws and regulations governing practice and continually updates knowledge base.
  • Able to multi-task and switch priorities throughout the day.
  • Excellent writing skills, and the ability to present facts and recommendations effectively in oral and written form.
  • Must be able to lift/move the equivalent of 25-50 pounds when necessary to provide care
  • Excellent interpersonal skills and ability to communicate effectively with individuals with disabilities, families, co-workers, staff, supervisors, funding sources, and other social service agency personnel
  • Ability to solve problems effectively
  • The ability to work independently with minimal supervision

Essential Responsibilities:

  • Supervision of LPNs, or others providing nursing care to individuals
  • Provides feedback, performance reviews, and assists with hiring of nursing staff.
  • Assist with budget development for medical care clients
  • Provide medical training for all staff when needed.
  • Other job duties as assigned
  • Monitor the consumer’s use of medications through Medication Administration logs.
  • Make referrals for needed medical services and confirming that follow-up care is rendered
  • Participates in initial evaluations by securing and reviewing all pertinent medical records and assessing physical well-being of residents
  • Adheres to standard health precautions and ensures wellbeing of residents by monitoring individual health, administering medication as prescribed, following up on medical, dental and emotional health requirements
  • Engage residents in communication, social, recreation, and ADL skills, financial, training, habilitation, and family/community functioning; reports observed changes in health or behavior to supervisor; all nursing tasks are done under the medical supervision and delegation of the Clinical Director.
  • Administration of tube feedings and care of G-Tube site and care associated with all designated medical procedures including, but not limited to, oral or nasal suctioning, bladder or supra pubic catheterizations and all other medical needs of individuals.
  • Administers prescribe medication to consumers and first aid to consumers and/or staff and prepare documents or progress notes relating to medication administration.
  • Drives van or other vehicle Accompany residents on all medical appointments or to hospital during scheduled shifts as Tender Cares person of medical expertise, follow through on all medical orders and coordinate follow up appointments as needed.
  • Assist in areas such as shopping, meal preparation, cleaning, bathing and dressing, transferring, toileting, feeding, grooming and hygiene; supports individuals in implementation of IHP/ISP or other goals and objectives; performs household chores as needed.
  • Drives van or other vehicle; escorts residents to scheduled programmatic or other activities, recreational opportunities, or medical appointments as assigned by supervisor.
  • May interact with residents’ family members and friends and facilitate communications.
  • Attends and participates in staff meetings may assist residents to participate in house meetings.
  • Adhere to DDD and Tender Cares circulars, policies and procedures relating to health and safety and participates in fire drills.
  • Performs other related tasks as assigned by Clinical Director

Physical Requirements:

  • Mobility and ability to bend and reach during an eight-hour day. Able to lift a minimum of 25 pounds. Able to sit approximately 2-3 hours per shift, stand or work 6-8 hours per shift, and drive a company van 1-4 hours per shift. Has visual and auditory acuity sufficient to evaluate and treat a client's needs. Fine motor skills and legible and accurate writing of reports, daily correspondence and presentation either manually or orally. Ability to work an eight-hour day.

Working Conditions:

  • Position is based at various locations in New Jersey. Incumbent must be willing to work a variety of hours, including evenings and weekends.
