Education

Bachelors Degree : in Nursing (Required)

Masters Degree : in Nursing (Preferred)

Experience

3 Years experience in disease or case management services with focus on telephonic management, medical reconciliation and ambulatory care coordination (Required)

General Experience with clinical, hospital-based information systems (Required)

License/Certifications

RN-LIC - PA Registered Nurse License (Required)
