Seeking a full-time employee as a Case Manager to work as part of multidisciplinary team for our Full Service Partnership (FSP) program in Woodland Hills. This program serves individuals aged 18 and older, who, as a result of their diagnosis of severe and persistent mental illness, have recent or current experience of homelessness, incarceration and/or hospitalization. The multidisciplinary treatment team works closely together to provide intensive services to promote rehabilitation and reintegration into the community. Case Managers are an important part of that multidisciplinary team that works together to help individuals on their path to obtaining and maintaining recovery and wellness.
Duties:
Bi-lingual Spanish/English preferred
BA/BS/BSW degree in social work or related fields or 3 years prior case management experience
