Job Description

The Care Team Coordinator (CTC) is at the core of Tapestry 360 Health’s (Tap 360) team-based care model. The Care Team Coordinator (CTC) participates on a care team with a provider and one other CTC, working together to meet patient needs. The role is patient-facing and responsibilities include setting visit agendas, gathering patient information, and performing vital signs. Other tasks include documentation support, facilitating communication between the patient and the rest of the care team, and ensuring patient understanding of their care plan. The CTC is critical in building trusting, supportive, and healing relationships with patients. The CTC role is an ideal position for someone who wants to work (and learn!) in a team environment and is passionate about helping to transform healthcare. Visit-based skills and experience will be gained through the onboarding process and continuous coaching.

Essential Duties & Responsibilities:

Visit-Based Patient Care

Pre-Visit Planning/Preparation

  • Reviews clinic schedule at the beginning of the day with care team to plan for each visit
  • Reviews patient’s medical record before visits to communicate results of medical reports to provider

Intake

  • Greets patient/families in waiting area and rooms them
  • Performs vital signs
  • Works with patient to set the visit agenda
  • Administers point of care testing (fingersticks, urine dips, EKGs)
  • Performs screening questionnaires such as anxiety/depression screenings

Team Visit

  • Briefs provider with visit agenda and other pertinent information
  • Provides documentation support throughout the visit
  • Prepares pending orders (referrals, labs, medications) under supervision of clinical provider
  • Ensures provider is clear in their explanations to patient

Discharge

  • Reviews care instructions from team visit with patient
  • Schedules follow-up appointments, as indicated
  • Coordinates care with other care team members
  • Assists with referral coordination
  • Discusses pending labs, administer vaccinations and medications as ordered by providers

Care Coordination

  • Acts as a liaison between patients and staff as well as other health organizations
  • Communicates with patients about prescription refills, laboratory results, and pending referrals

Population Health

  • In partnership with providers, monitors whether patients are current on preventive care measures
  • Completes patient care activities and uses population management tools for patient outreach

Relationship Building

  • Fosters continuity of care for patients by building connections with them across visits
  • Engages with patients to develop understanding and trust
  • Builds trusting relationships with other care team members

Operational Responsibilities

  • Ensures general upkeep of the clinical area
  • Completes inventory and supplies management
  • Helps support front desk and participates in daily health center operations
  • Facilitates huddles
  • Performs other job-related duties as assigned

Improvement / Innovation

  • Contributes to care team meetings by listening to and sharing ideas about process improvements
  • Participates in ongoing quality initiatives

Qualifications:

Education and/or Experience :

One of the following is required:

  • Medical Assistant diploma/certificate,

or

  • EMT-Basic diploma/certificate, or At least one (1) year prior experience as a Medical Assistant in a clinical setting

or

  • At least one (1) year prior experience working in a frontline healthcare role
  • Healthcare-related Associate degree and/or equivalent experience in health education or health related field desired.
  • At least six (6) months of experience working in a team-based care model preferred.
  • Experience with data entry, word processing, medical records and/or medical terminology preferred.
  • Bilingual English/Spanish preferred.
  • Current American Heart Association or American Red Cross CPR certification required upon hire and throughout employment.

Competencies:

  • Successfully completes required competency testing annually and as necessary.
  • Ability to use critical thinking and problem-solving skills to resolve job responsibility issues
  • Accountability for knowing and actively participating in quality initiatives.
  • A working knowledge of medical terminology and new developments in the health care delivery system, including federal and state health care regulations.
  • Familiarity with Centricity EMR preferred.
  • Ability to use software to accurately enter data, and receive, organize, prioritize, and transmit information in a timely manner.
  • Ability to implement organization policies and procedures and maintain compliance with established risk management and safety procedures, including patient privacy through HIPAA.
